A Photo ID

A Referral, If You Have One

A referral is helpful but not always required. Call us and we’ll help determine your plan’s requirements.

Your Copayment, If Applicable

Copays are expected at the time of service. Please ask our team if cost is a concern.

Your First Appointment

Your initial evaluation is typically 60–90 minutes. We take the time to understand where your pain is coming from, how it affects your life, and what a whole-body, root-cause approach looks like for you. Please wear comfortable clothing that allows us to assess your movement freely—and bring your questions.

A few helpful details before you complete your paperwork.

The packet asks for contact and emergency information, details about your primary complaint and therapy goals, medical history, medications and allergies, insurance or payment information, and authorizations for treatment and privacy practices.

Leave it blank and your therapist will assist you. The goal is to help us provide a complete and thorough evaluation using the information you can provide.

Medicaid and Medicare patients need a referral. For most other plans, a referral is not always required. Some insurance companies require one for reimbursement, so call our office at 801-685-9212 and we will help check before your visit.
